It is a mandatory pre-contract notice provided by the Washington Department of Labor & Industries (L&I). It informs property owners about a contractor’s registration status, bond information, and the potential for mechanics liens if subcontractors or suppliers go unpaid.
General contractors or direct contractors must provide this statement if they are working on residential projects (four or fewer units) with a contract price of $1,000 or more, or commercial projects with a contract price between $1,000 and $60,000. It is required when contracting directly with the property owner.
The disclosure statement must be signed by the property owner prior to starting work on the project. Contractors are required to retain a signed copy of this document for a minimum of three years.
